Medication Therapy Management Program

We offer a Medication Therapy Management Program to Eligible Members. This program is designed for those members who are at risk of reaching the Medicare Part D coverage gap or "donut hole". This program is for members who take many prescriptions, have many medical conditions or have high cost medication costs.

Certain types of medications and medical conditions qualify a member for the program. The purpose of the MTM Program is to keep our members out of the coverage gap, or at least delay the time it takes them to reach it.

Our clinical pharmacist reviews a member's list of medications using pharmacy records and then contacts the PCP to offer alternatives that may lower medication costs. Our PHD pharmacist will only change medications if the PCP approves the recommendations.
